Majestic Long-Haired with a Complex Health Profile: What Persian Cats Cost at the Vet
The Persian is one of the oldest pedigree cats in Europe. Its extreme brachycephaly (short-headedness) and dense long-haired coat fundamentally shape its health profile. Unlike other cat breeds, the financial peak values for Persians are notably high – particularly in old age.
Musculoskeletal: The absolute cost leader
Musculoskeletal is the most common category at 36.1% of all cases. The maximum payout in our data is €11,862 – the highest single value in our entire cat database. The trend across life stages is clear:
- Kittens (0–1 year): musculoskeletal 34% – early diagnostics, often still moderate costs
- Adults (2–7 years): musculoskeletal 36.4% – stable frequency, gastrointestinal diseases at avg. €564 per case more cost-intensive here
- Seniors (8+ years): musculoskeletal 66.7% – at avg. €5,993 per case the absolute peak. Complex procedures and lengthy orthopaedic treatments.
What this means: Anyone insuring a Persian cat should choose a high or unlimited annual benefit. Standard limits of €1,500–€3,000 can be exhausted by a single senior musculoskeletal case.
Brachycephalic Syndrome: Important insurance limitation
Persian cats are among the brachycephalic breeds – their shortened muzzle leads to several typical problems:
- Narrowed upper airways and possible breathing difficulties
- Tear duct displacement and constantly watery eyes
- Dental misalignments and jaw problems
- Facial fold pyoderma (skin inflammations in facial folds)
- Eye ulcers and corneal erosions
Important limitation: Brachycephalic syndrome is explicitly excluded at Dalma as a congenital condition. Acquired secondary conditions not directly caused by brachycephaly – e.g. an acquired facial skin infection from external causes – may be covered after waiting period. For many typical Persian problems (breathing distress due to anatomy, tear duct issues), there is no insurance protection.
Other common conditions
- Polycystic Kidney Disease (PKD): the central inherited disease in Persians – detected in one third of all Persian and Exotic Shorthair cats in the 1990s. Symptoms often only appear around the 7th to 8th year of life. Diagnostics via ultrasound approx. €100–300. Note: PKD is excluded as inherited condition.
- Hypertrophic Cardiomyopathy (HCM): above-average frequency in Persians. Diagnosis via cardiac ultrasound approx. €150–400. Note: HCM is excluded as a genetic feline condition.
- Progressive Retinal Atrophy (pd-PRA): inherited eye disease. Note: Excluded as inherited condition.
- Gastrointestinal diseases: for Persians avg. €559 per case. Maximum payout €2,347. Covered after 30 days.
- Skin diseases (facial fold pyoderma): treatment approx. €100–500. Acquired skin diseases covered after 30 days, provided not directly caused by brachycephaly.
- Eye problems (entropion, corneal erosions): treatment approx. €100–500. Acquired eye problems covered; congenital forms excluded.
- Dental diseases (FORL, gingivitis): often complicated by jaw misalignment in Persians. Covered after 12 months.
- Obesity and secondary conditions: Diabetes treatment approx. €300–800/year.
The cost ranges listed above are based on the German Veterinary Fee Schedule (GOT 2022) and typical market prices in German veterinary practices. Actual costs may vary depending on the practice, the scope of treatment and the chosen GOT rate (1x to 4x). Sources are listed at the end of this page.

What Is Not Covered for Persian Cats
Persian cats have several relevant breed-typical inherited conditions and anatomical problems excluded as congenital or genetic diseases:
- Brachycephalic syndrome – central anatomical issue of the Persian, explicitly excluded
- PKD (Polycystic Kidney Disease) – central Persian inherited disease, found in one third of the breed in the 1990s
- HCM (Hypertrophic Cardiomyopathy) – the most common genetic heart disease in cats
- PRA / pd-PRA (Progressive Retinal Atrophy) – inherited retinal disease
- Congenital eye diseases (e.g. primary entropion, corneal sequestrum)
- Persistent deciduous teeth
- Epilepsy
- Osteoarthritis
- Diseases preventable by vaccination (cat flu, feline distemper, leukaemia, rabies)
Acquired heart, kidney, eye and joint problems not directly attributable to brachycephaly or another inherited condition may be covered after waiting period. Due to the many breed-typical exclusions, it is particularly important for Persian cats to clarify existing pre-existing conditions before signing the contract.

Persian Insurance: Who Benefits Most
Choose high annual benefit: With a documented maximum payout of €11,862 for musculoskeletal treatments, an unlimited annual benefit is strongly recommended. Standard limits can be exhausted by a single senior case.
Insure early: Conditions arising after contract start are considered acquired. Insuring at kitten age means all waiting periods have long expired by the time of expensive senior treatments.
Realistic expectations: The central breed-typical problems – brachycephalic syndrome, PKD, HCM – are excluded as congenital/inherited conditions. Insurance protection applies to acquired conditions not directly attributable to these anatomical or genetic issues.
Less value: If your Persian cat already has a PKD, HCM or brachycephaly-related diagnosis before contract start. A medical assessment of previous diagnostics before contract signing is advisable.
A calculation: A senior Persian with one musculoskeletal treatment (avg. €5,993), one gastrointestinal treatment (avg. €559) and one skin treatment (avg. €458) reaches €7,000 in covered costs in a single year.
